Missions like Right Hand of Zazzmatazz, Silence the Informant, The Score... etc... all have named (are they truly officers?) rats that you have to kill. Does that mean these named rats have a chance (albeit low) to drop faction/officer mods?

All rats have change of dropping faction loot... it just that miniscule that you shouldn't get you hopes up on any particular rat.

Outside of storylines, the only missions with a chance to drop faction loot are Worlds Collide (from overseers) or Blockade (if you're lucky enough to get a faction rat, I've only ever seen one in the Serpentis version).

Absolutely false.
Then it seems that all those faction that I have gotten with my mission runner have been illusion. I loot every wreck in mission and in general got faction loot once in about 200 missions or so (not including blockade and WC).
EDIT: Wet Ferret: If you just loot some of your rats while you mission, I wouldn't be so firm on saying what there can be in wrecks or not

Mordus headhunters has a chance to spawn a faction boat in addition to the alreadymentioned WC and Blockade. When you say "fanboi" try to picture a fat man doing burlesque with 2 big ass fans that say CCP on one and HTFU on the other. Because that dude is me. |

I have pulled a piece of faction loot out of a wreck in a lvl 3 mission. although I think that I was just REALLY lucky to get that.

In general I find that the missions themselves don't matter very much. You just have to look at the missioners ship. When you suicide gank a tengu, you're more likely to get a faction of plex drop than if you gank a vanilla raven. This also drastically increases when you look at faction ships such as vindicators, nightmares, and machariels.
If you're looking for specific loot, for example true sansha or equivalent heat sinks, you want to gank amarr mission runners, as they often use lasers.
Certain ships, however, can be misleading. You might think a vindicator is armor tanked, and same for a mach due to them having more lowslots, but carebears are devious bastards. These ships are often shield tanked, giving them less EHP so theyre often easier to gank, and ofc, drop shield mods.
