Going by the obvious, it should do what it says on the tin. But, it doesnt actually seem to work.
I decided to sit on a gate in low sec, see if I could catch a few ships. And, targets appeared yet the module did not lock anything.
So I read the description, just to make sure..
"Targets any hostile ship within range on activation. Grants a +3 bonus to ship's max targets when online"
Hmm, ok maybe it only targets hostiles being negative standings. So after a little waiting, a negative show's. Low and behold, nothing happens. So what exactly does the Auto targetter do? If it waits for you to be targetted, then its no differant to just going to the ESC menu and turning on the "Auto target back option"
or have I missed something?

By "hostile" it basically means "already shooting at you"
Maybe they could be global criminal flagged or flagged to you / your corp, but I'm not sure exactly how it works.
It's pretty useless anyways.

When something shoots you with the autotargeter turned on it will automatically target them back up to your lock limit. If you have a free high and not enough fittings for anything else then a t1 AT is nice for quickly locking up aggressing drones in pvp situations. It does have a fairly hefty cap use so you only want to turn it on to get the locks then turn it back off. Be aware than manually targeting something before the AT finishes the lock cycle will cancel all autotargets in process.

But then the module is no differant to the check box option in your ESC -> General Settings "At bottom" Auto Target back option...

The esc menu option will auto target anyone locking you whilst the module only starts the process when they shoot you. This makes it handy for certain pvp situations like dealing with drone spam.

It may have some value for you in PVP but for PVE it is worse than worthless. I've had it target evil gas clouds ... and various other things. I spent more time un-targeting things it had targeted than I would have spent targeting the things I specifically did want to shoot at.
In PVE when you've got a dozen rats firing at you - it is a positively negative module to have turned on.
So - I took it off.

You still get the +3 targets benefit even when it is switched off provided you have the skills to lock that number of targets. If you have the skills to benefit from this then it is useful on some L4 mission ships.
